To ensure your child can benefit from the support offered by PROUD, we kindly ask that you complete the relevant referral and intake packet. These forms allow us to collect essential information about your child's needs and background, enabling us to provide the most suitable services. A downloadable version is available for your convenience, which you can easily fill out and submit online. Once we receive your completed packet, our team will review the information and reach out to you to begin the intake process. We are committed to responding promptly to all referrals and aim to complete the intake process as quickly as possible.
